Procedures & Reporting
for Health Care Providers
Office (828) 694-6019 | FAX (828) 697-4504
We follow protocols established by the Centers for Disease Control and the North Carolina Division of Public Health. Protocols for disease investigation and statistical data for North Carolina and Henderson County are available on the Division's website.
With the rollout of the North Carolina Electronic Disease Surveillance System (NCEDSS), the NC Communicable Disease Manual has been revised with reporting forms, compatible with NCEDSS, which replace previous reporting cards and surveillance forms.
Use the Confidential Communicable Disease Report—Part 1 in place of the morbidity cards.
Physicians or their designee are to complete the one page form and then FAX to 828-697-4504.
The form may also be sent by mail to the Henderson County Department of Public Health, Communicable Disease Control Section. The mailing address is printed on the Disease Report form.
Here are all currently reportable diseases with the required reporting time, 24 hours or 7 days. The complete revised North Carolina Communicable Disease Manual is available on the General Communicable Disease Control website.
For additional information on communicable disease reporting, please contact the Communicable Disease Control.
