Clinic hours are 8:00 AM - 4:30 PM, Monday through Friday.

You may call any day, Monday through Friday, starting at 8:00 AM to make an appointment.

On the third Wednesday of the month, we are closed in the morning for training. On that day, we start accepting calls for appointments at 11:00 AM.

Appointments (en Español) are made for the day that you want to be seen. To obtain an appointment, call the health department early in the morning and ask for the appointment you need. We have a limited number of appointments so the earlier you call the better your chances are of getting an appointment. Your phone calls are answered in the order they are received. Please do not hang up. When you call again, you will go to the back of the line.

Some appointments are pre-scheduled:

• Prenatal appointments
• Provider requested return visits
• Physical Exams

Physical Exam appointments for Child Health and Family Planning can be scheduled up to a week in advance. For these appointments starting on Monday, you can call at any time of the day to schedule an appointment for any day that week.

If you or your child is sick and need an appointment to be seen, but the schedule is already full when you call, please ask to speak to a nurse.

If you walk in without an appointment, you will be asked to use our telephone to make an appointment. This assures that everyone is able to make an appointment in the order that they call. If an appointment is not available, please ask to speak to a triage nurse.

Please come at the scheduled time. If you arrive late, we may have to reschedule your appointment. If you are unable to keep your appointment, please call ahead to cancel.

Payment and Proof of Income

If you have Medicaid or another health insurance, please bring your insurance card with you. If you have no health insurance, you can apply for our sliding fee scale which is based on income and family size. To apply for the sliding fee scale, please bring proof of income for your household. We accept paycheck stubs, a letter from the employer stating hours worked and wages, or last year’s income tax form. Until we receive this information, we will charge the full amount for our services.

To Enroll in WIC

All new appointments should be made by calling the WIC office on the day you wish to come in. We accept calls Monday - Thursday starting at 8:00 AM.

You will need to bring in the following information to be certified:

• Individual being certified
• Social Security card and shot records
• Proof of identification: valid driver's license, SS card, school ID, Medicaid card, military ID, or birth certificate
• Proof of residence: recent water bill, electric, cable, gas, or telephone bill, current rental or mortgage agreement
• Proof of income: current Medicaid card, Work First eligibility or Food Stamp approval letter. If these do not apply, bring your last month paycheck stub or recent tax return for self-employment
• Bring WIC Folder (can be used as ID)

To schedule your next WIC appointment

Please check your vouchers to see when you will need to come in. Try to schedule your WIC appointment about 1 week before your last vouchers run out.
